-Facility Waiver (once per year) https://wynfarm.aidaform.com/liability-waiver-form

-Negative Coggins (in past 12 months)

-Proof of Flu/Rhino vaccine (in past 6 months)

After submitting your entry you will get a confirmation email with a link to upload your documents. Please submit your documents using that link if you and your horse haven't already shown at Wyn Farm this year. Take care of the paperwork early and breeze through check-in the day of the show!
